Injured in a Car Accident?

If you are injured in a car accident, you should seek treatment from an authorized health care provider such as a Chiropractor. Your Chiropractor will assess, diagnose and treat the injuries caused by the collision. Your treatments are pre-approved and fully covered after you submit the claim to your insurance company. In order to receive these benefits you must complete the AB-1 Form (Notice of Loss and Proof of Claim).

Before you are eligible to start your treatments, you need to have an Assessment done by your Doctor of Chiropractic. The Doctor will complete the AB-2 Form (Treatment Plan)for you. The administrative staff have the forms available to you or you could find them in our website and will submit them to your insurance company. You are entitled to receive (10 to 21) treatments. After those treatments are completed, you Chiropractor will Re-Assess your injuries and determine if you need additional treatments. Section B of your car insurance policy will cover up to a maximum of $1,000.00 for Chiropractic, $350.00 for Massage and $350.00 for Acupuncture.
